The Importance of Monitoring
During a monthly check-in call, the CCM nurse spoke with a patient about the importance of blood pressure monitoring and found that the patient was monitoring only on occasion. The nurse encouraged the patient towards daily monitoring with a written journal to track trends. The following month, when the CCM nurse checked in on this patient again, he stated that he had begun monitoring as suggested, and had discovered he was having some instances of elevated blood pressure. With the data charted over time, he was able to work with the physician to adjust his medication accordingly. He was very pleased with the education and encouragement.
